Ascension Living Alexian PACE to Open New Chattanooga Center, Expanding Access to Care for Older Adults

 

 

Ascension Living Alexian PACE announced plans to relocate to a new, expanded facility in Chattanooga, creating a modern, centralized hub for the Program of All-Inclusive Care for the Elderly (PACE) and strengthening support for older adults across Hamilton County.

Expected to open in fall 2027, the new 44,000-square-foot center at 1600 E. 23rd Street will bring all PACE services together under one roof, making it easier for participants to access coordinated care while remaining in the homes and communities they love.

The facility will feature a dedicated adult day program, including 3 day centers, a large therapy gym, medical clinic and pharmacy with specialized programming designed to meet the evolving needs of older adults.

“As more of our neighbors in Hamilton County age and require additional support, many want to remain in their homes and communities for as long as possible,” said Beth Herring, Executive Director of Ascension Living Alexian PACE. “This new center will allow us to better coordinate services, enhance the participant experience, and continue delivering the compassionate, person-centered care that is at the heart of our Mission.”

The new location represents a significant expansion from the current Chattanooga footprint, consolidating multiple departments, including transportation, memory support, and home care services, that are currently spread across separate facilities. By centralizing operations, the organization expects to improve care coordination, streamline services, and create a more seamless experience for participants and their caregivers.

PACE is an innovative model of care that helps older adults with chronic medical conditions remain independent and avoid nursing home placement. Through a dedicated interdisciplinary care team, participants receive personalized medical care and support services tailored to their individual needs.

Ascension Living Alexian PACE was the first PACE program established in Tennessee and serves older adults throughout the Greater Chattanooga area. Participants receive comprehensive physical, social, spiritual, and behavioral health support, including primary care, therapies, transportation, medication management, social services, caregiver support, and adult day services.
