Ascension Living St. Vincent PACE is Now Open

Ascension Living St. Vincent PACE is now open at 5435 W. Pike Plaza Road, Indianapolis. The Program of All-Inclusive Care for the Elderly (PACE®) is an innovative model of care that allows seniors with long-term care needs to remain living in their community for as long as possible, as independently as possible.

According to the National PACE Association, “While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 5 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care.” 

The 38,000- square-foot PACE building features natural light, a large therapy gym, adult day center, medical clinic, outdoor courtyard, pharmacy, and chapel. The new center also has a dedicated adult day program for those living with dementia featuring specialized programming and a secure separate courtyard.

Ascension Living St. Vincent PACE provides geriatric-focused medical and supportive wraparound care that help adults 55 or older with chronic medical conditions stay living in their home and the community they love. St Vincent PACE serves 47 zip codes within northern Marion County and surrounding areas. Participants benefit from a dedicated care team made up of 11 interdisciplinary providers, including a primary care physician, registered nurse and social worker, who create a personalized care plan for each unique participant. 

Through the new Ascension Living St. Vincent PACE program, PACE participants have access to physical, social, spiritual and behavioral health services; and support for their caregivers.  Benefits include adult day centers; transportation to the center and medical appointments; medication, home delivery and management; physical, occupational, speech and restorative therapies; pastoral care; social services and activities; and extra services based on their needs. PACE offers enrollees little to no monthly fees and no benefit limitations, copayments or deductibles.
